# Reporting a form

# Reporting a form

If something about a form seems abusive, deceptive, or harmful, respondents (and you) can flag it confidentially for review.

## How to report

A **"Report this form"** link appears near the form's footer, and a flag icon also appears in the voice runner's top bar — so a form can be reported at any time, even before answering. Reporting takes a couple of taps:

1. Pick a reason — like harassment, violence, fraud, or a privacy concern.
2. Optionally add a note with more context.
3. Send it confidentially to ChatWisp's Trust & Safety team.

## What happens next

ChatWisp's Trust & Safety team reviews every report. If a form is found to break the rules, it may be taken offline and its responses removed, and the creator is notified with a way to reach support.

As a creator, remember that abusive, deceptive, or harmful forms violate ChatWisp's policies. Build forms that respect your respondents and contribute to a trustworthy platform.

## Common questions

### Who can report a form?
Anyone who can open it. The control is on the respondent view itself, and it stays available even on forms that have removed ChatWisp branding.

### Do I have to be signed in to report something?
No. Reporting is open to any respondent, which is the point: the people most likely to encounter a problem form are not our customers.

### What gets sent with a report?
A frozen snapshot of the form exactly as it appeared: its questions, options and styling. The creator's private research notes are excluded from that snapshot.

### What can happen to a reported form?
Outcomes range from dismissal through a warning, taking the form offline, deleting it with its results, and suspending the account behind it. A person makes that decision every time.

### Will the creator know it was me?
No. Reports are not attributed back to the person who made them.
